A life care plan is a vital part of creating a path for one's future that outlines both the medical and non-medical needs of an individual over their lifetime related to their specific disability- whether from a catastrophic injury or chronic health condition. Over the past 18 years, I have practiced as a licensed occupational therapist in the long term care, acute inpatient rehabilitation, and outpatient therapy environments.

Complete life care plans to identify future medical and non-medical needs for individuals sustaining catastrophic or complicated injuries/illnesses. Serve as an expert witness in court as a Certified Life Care Planner. Referral resources include insurance carriers, attorneys, employers, and client/family members. 

 

Occupational Therapist

Roper Hospital, Charleston, SC, January 2010-present

Evaluate and treat individuals in the following settings:  acute inpatient rehabilitation, acute inpatient, intensive care unit, and outpatient.  Responsible for creating treatment plans for individuals with the diagnosis of traumatic and non-traumatic spinal cord injuries, traumatic brain injuries, cerebral vascular accidents, neurological disorders, orthopedic injuries, musculoskeletal injuries, amputations, and general deconditioning.  Responsible for co-signing and creating goals for COTAs.  Serve as a clinical instructor for OT students.  I am on the Spinal Cord Injury team where we make sure that our system is providing education and a smooth continuum of care for patients with SCI throughout the system. I also serve as an OT in the SCI clinic that we provide two times a month for those out in the community to educate and identify their needs.

Area Manager, Rehab Manager, OTR/L

Heritage Healthcare, Greenville, SC, June 2003-January 2010

Began as a manager at a skilled nursing facility for a rehabilitation company.  Duties included scheduling, attending daily meetings discussing patient care plans, evaluating and treating patients, and being the liaison between the facility and the rehabilitation company.  Moved into the Area Manager role and managed up to eight rehab managers in the lowcountry of South Carolina, Georgia and Alabama.  This role included chart reviews/audits, staffing and training, corporate meetings for company goals, presenting quarterly meetings for staff, evaluating and treating patients in an Occupational Therapist role.
